I did it, but when they asked for details, I could not for the life of me remember them. They turned me down because apparently I'm a phony if I can't remember that. This happened about 15 years ago, but I still remember it.

Most document repositories have a notes section or a description; deliverables over email have message bodies (often ignored). Documents can have tracked changes and annotations. There's a whole ecosystem of comments and commit messages in the PM world too! </epiphany>
