I've set up a bendy bone between two handles. it looks fine in rest position:

but as soon as I try to pose the top handle, even just moving it a little bit, it gets twisted:

You need to keep the main bone parented to the Start bone, and also make sure that the main bone’s head sticks to the Start bone tail, which is not the case for the moment because the Start bone is inverted, it messes up the axis.

To fix that, in Edit mode select the Start bone and RMB > Switch Direction, then parent the main bone to the Start bone. Result:
